Any individual who is terminally ill or nearing the end of life. No age limit, no diagnosis requirement, no financial qualification, and no hospice enrollment required.
WHO QUALIFIES
Where We Perform
Private homes, hospice houses, hospitals, skilled nursing, assisted living, memory care and residential care homes. If there is room for two musicians, there is room for a concert.
What We Handle
Musician matching and booking, scheduling with the family or facility, a trained concert liaison on site, and all musician honorariums. There is no cost to the patient, the family or your organization.
What We Need From You
The patient’s music preference, a contact for scheduling, and a realistic sense of timing. That is genuinely it.
How Often You May Refer
As often as you like. No cap, no waiting period. A second request for the same patient is welcome if circumstances change, and so is a request next week for a different patient.
WHEN A CONCERT ISN’T POSSIBLE
Pre-recorded and Virtual Concerts
Where health or safety concerns prevent a live concert — isolation precautions, infection control, immunocompromise, or a facility restriction — we can create a custom pre-recorded or virtual concert the family can keep and share afterward.
Note this on the referral and we will plan for it from the start. If a patient’s status changes between referral and performance, tell us and we will adapt.

1 Submit the Request
Use the online form and select your location. Include the music preference and the best contact for scheduling.
2 We Confirm With You
Our program team contacts you or the family directly, usually within two business days, and sooner when you have told us time is short.
3 We Schedule and Arrive
You do not need to coordinate musicians, arrange payment, or be present — though you are always welcome to attend.
